Diabetes Specialist Nurse Opportunity

We are seeking a highly skilled Diabetes Specialist Nurse to join our team at our GP Practice. As a vital link between primary and secondary care, you will play a crucial role in preventing unnecessary hospital admissions and enhancing life quality for patients with diabetes.

As a Diabetes Specialist Nurse, you will be responsible for providing comprehensive and specialized care to patients with diabetes, including assessing and analyzing complex needs, advising on appropriate interventions, and monitoring and evaluating treatment plans.

You will work closely with our multidisciplinary team to deliver high-quality care and support to patients, their families, and carers. Your expertise will be essential in promoting health and preventing ill health, and you will be a professional advocate for patients with diabetes within the healthcare setting.

Main Responsibilities
  1. Provide specialist diabetes advice and care to patients with complex needs.
  2. Assess and analyze patient needs, and develop and implement individualized treatment plans.
  3. Work collaboratively with the diabetes team to deliver high-quality care and support.
  4. Participate in clinical treatment and management reviews of patients with diabetes.

Person Specification
  • Experience of working in a diabetes care setting.
  • Strong clinical skills and knowledge of diabetes management.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Excellent communication and interpersonal skills.
  • Willingness to acquire new skills and knowledge and undertake further training as required.
