About the Role

We are seeking a highly skilled and compassionate Lead Support Worker to join our Hounslow Reach team. As a key member of our team, you will play a vital role in empowering individuals to achieve their aspirations and build independence.

Key Responsibilities
  • Work collaboratively with individuals who have experienced hospitalization to develop skills, confidence, and aspirations, reducing social isolation through community engagement.
  • Establish and maintain productive partnerships with clinicians, social services, housing, and community stakeholders to source optimal outcomes for individuals leaving hospital.
  • Reflect, record, and report on your work to inform service development and improvement.
  • Exercise initiative and problem-solving skills to navigate complex systems and access necessary services.
  • Provide respectful, trauma-informed support to individuals, promoting their well-being and dignity.
  • Work effectively within a team to achieve shared goals and objectives.
Requirements
  • Experience in empowering individuals to achieve aspirational outcomes and build independence.
  • Creative and proactive problem-solving skills.
  • Ability to work collaboratively as a positive and effective team member, achieving shared goals.
  • Experience in developing and sustaining effective working partnerships with stakeholders, particularly in health and housing.
  • Organizational and administrative skills, including managing competing priorities, communicating effectively, and utilizing various IT systems.
  • Confidence in working autonomously within the community, in individuals' homes, and hospitals.
