Youth Independence Coordinator

Providing Care is seeking a dedicated Youth Independence Coordinator to join our team. As a coordinator, you will play a crucial role in empowering young adults to achieve independence and reach their full potential.

Your key responsibilities will include:

  • Developing and implementing individualised support plans tailored to the unique needs of each young person.
  • Building strong relationships with young people, social workers, and other relevant parties to ensure effective coordination of care.
  • Providing emotional support and guidance to help young people overcome challenges and achieve their goals.

Requirements:

  • Experience working with vulnerable populations, particularly young people with complex needs.
  • Knowledge of local community resources and services.
  • Commitment to promoting equality, diversity, and inclusion.
