Job Title: Nurse Associate

We are seeking a highly skilled and compassionate Nurse Associate to join our Transformation Unit team. As a Nurse Associate, you will play a vital role in delivering high-quality, patient-centered care to older adults, ensuring their physical health needs are met.

Main Responsibilities:
  • Deliver Comprehensive Care: Conduct thorough nursing assessments, phlebotomy, and vital sign checks, while developing person-centered plans that address physical health risks and needs.
  • Work Independently: Collaborate with registered nurses, working to their nationally recognized code of conduct, and complying with trust policies and procedures.
  • Develop Physical Health Clinics: Help design and deliver clinics focused on compassionate and personalized care, working closely with the multidisciplinary team.
  • Administer Tasks: Record clinical data, communicate with primary and secondary care services, and monitor patient conditions and health needs.
  • Recognize and Report Risks: Identify potential safety risks and take appropriate action, prioritizing the best interests and needs of patients.
  • Support Patient Wellbeing: Assist in implementing actions to meet patients' physical, emotional, and psychological needs, promoting overall health and wellbeing.
Requirements:
  • Education and Qualification: Registered Nursing Associate on the NMC register.
  • Community Experience: Working with older adults with mental health problems.
  • Facilitation Skills: Experience in facilitating group activities and social inclusion.
  • Travel and Flexibility: Ability to travel to NHS and Local Authority sites, and patients' homes, with a valid driving license and use of a vehicle.
