About the Role

Oxleas NHS Foundation Trust is seeking a skilled Nursing Associate to join our team in delivering high-quality, compassionate care to older adults. As a Nursing Associate - Physical Health Specialist, you will play a vital role in ensuring the physical health needs of our patients are met.

Key Responsibilities
  • Develop and deliver physical health clinics with a focus on compassionate and personalized care.
  • Conduct comprehensive physical health assessments, including vital signs and medical history.
  • Assess and manage risks associated with physical health, and develop person-centered plans to support patients.
  • Work independently under the leadership of registered nurses, using your skills and knowledge to provide high-quality care.
  • Communicate effectively with patients, families, and colleagues, and maintain accurate records of patient care.
  • Participate in multidisciplinary team meetings and contribute to the planning of care delivery.
  • Identify and report any situations that may compromise patient safety or well-being.
Requirements
  • Registered Nursing Associate on the NMC register.
  • Community experience of working with older adults with mental health problems.
  • Ability to travel to NHS and Local Authority sites and patients' homes.
