Job Title: Nurse Associate
Job Summary:
The Transformation Unit is seeking a skilled and compassionate Nurse Associate to join our team. As a key member of our healthcare team, you will play a vital role in delivering high-quality care to older adults, ensuring their physical health needs are met.
Main Responsibilities:
- Comprehensive Nursing Assessments: Conduct thorough nursing assessments, including phlebotomy, and vital sign checks, to identify the physical health needs of older adults.
- Person-Centred Care: Develop and implement person-centred plans, taking into account the individual's physical health risks and needs, to ensure they receive tailored care.
- Collaboration with Healthcare Teams: Work closely with the Bromley Older Adults Community Mental Health Team and other healthcare professionals to ensure seamless care delivery.
- Administrative Tasks: Record patient information, communicate with primary and secondary care services, and complete necessary paperwork.
- Risk Assessment and Management: Identify and report potential risks to patient safety, and take appropriate action to mitigate them.
- Communication and Teamwork: Communicate effectively with colleagues, patients, and families, and work collaboratively to achieve shared goals.
Requirements:
- Education and Qualifications: Registered Nursing Associate on the NMC register.
- Experience: Community experience working with older adults with mental health problems, and experience facilitating group activities and social inclusion.
- Skills: Ability to travel to NHS and Local Authority sites, valid driving license, and use of a vehicle.
