Oxleas NHS Foundation Trust is seeking a skilled Nursing Associate to join our team in delivering high-quality, patient-centered care to older adults. As a Nursing Associate - Physical Health Specialist, you will play a vital role in ensuring the physical health needs of our service users are met.
Main Responsibilities- Comprehensive Nursing Assessments
- Deliver thorough nursing assessments, phlebotomy, and vital sign checks, prioritizing compassionate and patient-centered care.
- Person-Centered Planning
- Support the development of person-centered plans for service users, taking into account their physical health needs and associated risks.
- Collaborative Care Delivery
- Work independently under the leadership of registered nurses, adhering to nationally recognized codes of conduct and standards of proficiency for Nursing Associates.
- Effective Communication
- Communicate effectively with service users, families, and carers, ensuring their privacy, dignity, and safety are maintained at all times.
- Health Monitoring and Risk Assessment
- Monitor the condition and health needs of service users, working in partnership with multidisciplinary teams, families, and carers.
- Nursing Risk Assessments
- Complete nursing risk assessments using recognized risk assessment tools and contribute to the planning of care delivery in association with registered nurses.
- Quality Improvement
- Recognize and report any situations, behaviors, or errors that could result in poor care outcomes and identify risks to safety or experience, taking appropriate action.
- Health Promotion and Prevention
- Support service users to improve and maintain their mental, physical, behavioral health and wellbeing, and be actively involved in the prevention of and protection against disease and ill health.

Person Specification- Education/Qualification
- Registered Nursing Associate on the NMC register.
- Essential Skills/Abilities/Knowledge
- Ability to undertake and understand physiological measurements and observations, pulse oximetry, and general observations.
- Ability to travel to NHS and Local Authority sites and patients' homes.
- Desirable Skills/Abilities/Knowledge
- Experience of facilitating group activities and social inclusion.
- Valid driving licence and the use of a vehicle to travel across a large borough.
