The Transformation Unit is seeking a skilled and compassionate Nursing Associate to join our team. As a key member of our healthcare team, you will play a vital role in delivering high-quality care to older adults, ensuring their physical health needs are met.

Main Responsibilities
  • Comprehensive Nursing Assessments: Conduct thorough nursing assessments, including phlebotomy, and vital sign checks, to identify the physical health needs of our service users.
  • Person-Centred Care Planning: Develop and implement person-centred care plans, taking into account the individual needs and risks associated with our service users' physical health.
  • Collaboration with Healthcare Teams: Work closely with our multidisciplinary team, primary and secondary care services, to ensure seamless care delivery and effective communication.
  • Administration and Record-Keeping: Accurately record and maintain clinical records, ensuring up-to-date information is available for healthcare professionals.
  • Risk Assessment and Management: Identify and report any risks to safety or well-being, taking appropriate action to mitigate these risks and ensure the best possible outcomes for our service users.
Requirements
  • Registered Nursing Associate: Hold a valid registration with the Nursing and Midwifery Council (NMC).
  • Community Experience: Possess experience working with older adults with mental health problems, including facilitating group activities and promoting social inclusion.
  • Travel and Flexibility: Be able to travel to various locations, including NHS and Local Authority sites, and adapt to changing priorities and deadlines.
