About the Role

Oxleas NHS Foundation Trust is seeking a skilled Nursing Associate to join our team in delivering high-quality, compassionate care to older adults. As a key member of our multidisciplinary team, you will work closely with registered nurses and other healthcare professionals to ensure the physical health needs of our patients are met.

Key Responsibilities
  • Develop and deliver physical health clinics with a focus on compassionate and personalized care.
  • Conduct comprehensive physical health assessments, including vital signs and medical history, and complete monitoring forms.
  • Take a thorough medical history, including family history and lifestyle issues, and follow up on national screening programs as needed.
  • Support in developing person-centered plans for service users, taking into account physical health risks and needs.
  • Work independently under the leadership of registered nurses, adhering to national codes of conduct and trust policies.
  • Communicate effectively with colleagues, patients, and families, and maintain confidentiality and dignity at all times.
Person Specification
  • Registered Nursing Associate on the NMC register.
  • Community experience working with older adults with mental health problems.
  • Ability to travel to NHS and Local Authority sites and patients' homes.
