About the Role
This is an exciting opportunity to work as a Nursing Associate in the Oxleas NHS Foundation Trust, specifically in the area of physical health for older adults. As a key member of the team, you will play a vital role in delivering high-quality care to our patients.
Key Responsibilities
- Develop and deliver physical health clinics with a focus on compassionate and personalised care.
- Conduct comprehensive physical health assessments, including vital signs and medical history, and complete monitoring forms as required.
- Support patients in developing person-centred plans, taking into account their physical health needs and any associated risks.
- Work collaboratively with the multidisciplinary team to ensure seamless care delivery and effective communication with patients and their families.
- Administer tasks, such as recording on clinical records and communicating with primary and secondary care, to ensure accurate and timely care delivery.
- Participate in group work, activities, and workshops, holding a small caseload, to promote social inclusion and community engagement.
- Work independently under the leadership of registered nurses, adhering to nationally recognised codes of conduct and standards of proficiency for Nursing Associates.
- Communicate effectively with sensitivity and compassion, managing relationships with people and making reasonable adjustments where necessary.
- Maintain the privacy, dignity, and safety of individuals at all times, monitoring their condition and health needs on a continual basis in partnership with the multidisciplinary team, families, and carers.
- Complete nursing risk assessments using recognised risk assessment tools and contribute to the planning of care delivery in association with the Registered Nurse.
- Recognise and report any situations, behaviours, or errors that could result in poor care outcomes, identifying risks to safety or experience and taking appropriate action.
- Assist in the implementation of appropriate action to meet the specific physical, emotional, and psychological, social, cultural, and spiritual needs of individuals and carers.
- Demonstrate a good understanding of principles of consent and ensure valid consent is obtained before undertaking nursing and care procedures.
- Support people to improve and maintain their mental, physical, behavioural health, and wellbeing, being actively involved in the prevention of and protection against disease and ill health.
- Work flexibly to meet critical short-term and long-term deadlines, acting sensitively at all times to ensure that concerns of staff, patients, and relatives are dealt with sympathetically.
- Use frequent moderate effort when undertaking manual handling of individuals and equipment in line with organisational guidelines.
- Use skills of manual dexterity and manipulation of clinical equipment.
- Use a combination of standing, walking, and bending throughout the shift.

Person Specification
- Registered Nursing Associate on the NMC register.
- Community experience of working with older adults with mental health problems.
- Experience of facilitating group activities and social inclusion.
- Ability to undertake and understand physiological measurements and observations, pulse oximetry, and general observations.
- Ability to travel to NHS and Local Authority sites and patients' homes.
- Valid driving licence and the use of a vehicle to travel across a large borough.
