We are seeking a dedicated and compassionate Registered Nurse to join our community-based care team at the Kent Community Health NHS Foundation Trust.

The successful candidate will work to promote the health and wellbeing of individuals with learning disabilities and their families/carers, providing direct specialist clinical expertise and behavioral support.

Our community nursing teams closely collaborate with local Primary Care Networks to ensure comprehensive and coordinated patient care, facilitating reasonable adjustments to enable people to access mainstream healthcare services.

Main Duties

The multi-disciplinary teams, comprising Occupational Therapists, LD Nurses, Speech & Language therapists, Physiotherapists, and a specialist vision and hearing service, operate across five geographic localities within the County.

They collaborate with social care teams from Kent County Council (KCC) and LD teams within the Mental Health Trust (KMPT) to provide a coordinated and holistic service for individuals with learning disabilities.

Person SpecificationRequirements
  • A minimum of 3 years of community nursing experience
Qualifications
  • A health-related degree and evidence of relevant postgraduate development at level 6
Desirable Skills
  • Managing health-related projects and complex caseloads

Estimated salary: £40,000 - £60,000 per annum, dependent on experience and location.
