Job Description

We are seeking a dedicated and motivated Health Visitor to join our team in the High Peaks, North Dales, and Glossop locality.

The successful candidate will have a strong background in public health and a passion for delivering high-quality healthcare services to families and communities.

The role will involve managing a defined caseload and taking responsibility for the assessment of care needs, the development, implementation, and evaluation of programs of care.

The ideal candidate will be a registered nurse with a qualification in Health Visiting and a strong understanding of the Healthy Child Programme.

Main Responsibilities

  • Deliver health visiting services to families in the community team caseload by implementing the Healthy Child Programme.
  • Manage a defined active caseload and take responsibility for safeguarding children issues within that caseload.
  • Continuously assess individual needs and develop, implement, and evaluate programs of care.
  • Manage a team of health visiting support staff.
