About the Role
We are seeking a skilled and dedicated Health Visitor to join our team at Central London Community Health Trust. As a Health Visitor, you will play a vital role in delivering high-quality health visiting services to clients in a variety of community settings.
Key Responsibilities
- Provide skilled, effective, and evidence-based health visiting services to clients.
- Apply specialist skills and knowledge in the delivery of the health visiting service.
- Consult, advise, and educate individuals and families on good health care practices.
- Develop health profiles of the defined population and local communities, identifying health needs of disadvantaged groups and targeting outreach services to meet their needs.
- Identify, prioritize, and implement programs of care that meet the public health needs of the population.
- Provide parenting skills support and information through coordinating/participating in parenting groups and one-to-one home visits to families, especially those with complex health needs/vulnerabilities.
- Provide health assessments, advice, and education to families and individuals of all ages in the population group, prioritizing services appropriate to local public health and population needs.
Person Specification
- Current NMC registration is essential.
- Evidence of continuing post-registration education and training is required.
- Recent health visiting experience is necessary, with the ability to coordinate health promotion initiatives in line with local and national health improvement targets.
- Experience of assessing, planning, and implementing programs of care and writing support for others is desirable.
