Job Summary

We are seeking a highly skilled and experienced Health Visitor to join our team at Central London Community Health Trust. As a Health Visitor, you will play a vital role in promoting the health and wellbeing of children and families in our community.

About the Role

This is an exciting opportunity to work in a dynamic and supportive team, providing high-quality health visiting services to families in Hammersmith and Fulham. You will be responsible for delivering the Healthy Child Programme, working in partnership with families to develop and agree tailored health plans, and utilising your specialist skills and knowledge to advise and support individuals and families on evidence-based healthcare practices.

Key Responsibilities
  • Deliver the Healthy Child Programme and work in partnership with families to develop and agree tailored health plans
  • Utilise specialist skills and knowledge to advise and support individuals and families on evidence-based healthcare practices
  • Understand the caseload profile, including the health needs of the defined population and local communities, and target outreach services to meet their needs
  • Initiate referrals to multi-disciplinary/agency colleagues as required to ensure adequate care and support for families and children
  • Assess, plan, implement, and evaluate individual/group programmes of care that meet the public health needs of the population
Person Specification

We are looking for a registered nurse or midwife with a strong background in health visiting, who is able to work independently and as part of a team. You will have excellent communication and interpersonal skills, and be able to adapt to changing circumstances and priorities.

Essential qualifications and experience include:

  • Registered Nurse or Registered Midwife
  • Registered Health Visitor (SCPHN)
  • Current NMC registration
  • Evidence of continuing post-registration education and training
  • Practice assessor/supervisor training or working towards
