About the Role
We are seeking a skilled and dedicated Health Visitor to join our team at Central London Community Health Trust. As a Health Visitor, you will play a vital role in promoting the health and wellbeing of families and children in our community.
Key Responsibilities
- Deliver the Healthy Child Programme and work in partnership with families to develop and agree tailored health plans as required.
- Utilise specialist skills and knowledge to advise and support individuals and families on evidence-based healthcare practices.
- Assess, plan, implement and evaluate individual/group programmes of care that meet the public health needs of the population.
Requirements
- Registered Nurse/Registered Midwife qualification.
- Registered Health Visitor (SCPHN) qualification.
- Current NMC registration.
- Evidence of continuing post-registration education and training.
- Practice assessor/supervisor training or working towards.
