Job Title: Community Mental Health Practitioner

Livewell Southwest is seeking an experienced registered practitioner to join our Community Mental Health Team. The successful candidate will work as part of a multidisciplinary team, taking referrals, conducting biopsychosocial assessments, and providing treatment interventions for patients experiencing mental health crisis.

Main Duties:
  • Receive and triage referrals for individuals presenting in a mental health crisis
  • Participate in a multi-disciplinary team approach responding to individuals requiring a same-day response or signposting due to a mental health crisis
  • Provide skilled telephone advice to referrers and patients and carers
  • Offer an urgent mental health assessment in a location most convenient/appropriate for the patient
  • Identify the most appropriate intervention required for the patient, with an emphasis on positive risk management, and refer as necessary
  • Follow assessments, provide high-quality assessment reports/letters to relevant professionals, and monitor reports by junior staff to ensure they meet standards
  • Support the Team Manager in the effective running of the team and monitoring of standards through effective clinical supervision of junior staff
Requirements:
  • Significant post-registration experience of working with people with complex mental ill health within the community
  • Experience of working collaboratively with patients and their families/carers
  • Previous experience of working at a Band 6 level or 2 years post-registration experience in acute mental health
  • Completing complex mental health assessments in a variety of different settings in the community and signposting to relevant services
  • Relapse prevention and Recovery Principles
  • Experience of providing clinical supervision, mentoring, and teaching to students and other health professionals
