Are you a dedicated mental health professional looking for a new challenge? NHS Professionals Limited has an exciting opportunity for a Flexible Community Mental Health Practitioner to join our team in England.

In this role, you will provide robust assessments of patients who have been referred due to presenting with mental health difficulties. You will identify appropriate care pathways and ensure the least restrictive option to meet the needs of the patient and ensure their well-being.

You will also signpost to appropriate care providers, raise concerns to senior staff for support and management, complete documentation and planning care for patients, present cases to Multi-Disciplinary Team for discussion around care pathway, and liaise with colleagues.

**Requirements**: A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3 years, compliance with NMC Professional Registration, and possession of a full UK driving licence and own vehicle.
