Job Description:

Livewell Southwest CIC is seeking an experienced Mental Health Practitioner to join our team. The successful candidate will work closely with primary care and third sector agencies to provide high-quality care for individuals experiencing mental health problems in Plymouth.

Key Responsibilities:

  • Provide initial assessments and support calls within GP surgeries to determine eligibility for secondary mental health care services and interventions.
  • Deliver evidence-based recovery-focused interventions for patients on an individual or group basis in conjunction with partner agencies.
  • Provide consultation to primary care and third sector agencies to ensure timely referrals to appropriate services.
  • Work closely with Plymouth Primary Care Networks (PCNs) and GP surgeries to provide assessment, support, and guidance for individuals presenting frequently or with complex problems.
  • Support the development and delivery of training for patients, carers, and partner agencies.
  • Develop and maintain links with primary care and third sector agencies.

Requirements:

  • RMN/Dip HE/BSc Mental Health Nursing - NMC Registration.
  • Occupational Therapy Degree HCPC Registration.
  • Social Work Degree or PG Dip or MA Social Work England Registration.
  • Relevant experience of working with adults with a history of severe and enduring mental illness.
  • Experience within a community mental health setting.
