About the Role

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our Community Crisis Response Team. As a key member of our multidisciplinary team, you will play a vital role in providing crisis intervention and support to individuals experiencing mental health crises in the community.

Key Responsibilities
  • Receive and triage referrals for individuals in mental health crisis, providing timely and effective interventions to prevent escalation.
  • Work collaboratively with patients, carers, and other healthcare professionals to develop and implement personalized care plans.
  • Conduct comprehensive biopsychosocial assessments to identify patient needs and develop effective treatment strategies.
  • Provide skilled telephone advice and support to patients, carers, and referrers, promoting positive risk management and crisis resolution.
  • Offer urgent mental health assessments in community settings, such as patients' homes or GP surgeries, to provide timely and effective interventions.
Requirements
  • Significant post-registration experience working with individuals with complex mental health needs in community settings.
  • Ability to work effectively under pressure, prioritizing tasks and managing multiple referrals.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, carers, and healthcare professionals.
  • Knowledge of mental health legislation, policies, and guidelines, including the Mental Health Act 1983 and the Care Programme Approach.
  • Ability to work flexibly, including evenings and weekends, to meet the needs of the service.
