Role Overview

In this pivotal role, you will play a crucial part in supporting individuals with diabetes to achieve better health outcomes through tailored education, guidance, and behaviour change interventions. This position requires expertise in Behaviour Change (e.g., Motivational Interviewing, CBT, or other behaviour management techniques) as well as excellent Communication Skills, with the ability to build rapport and inspire patients to adopt and maintain behaviour changes.

Main Responsibilities:
  • Deliver support, education, and advice to individuals living with diabetes, ensuring optimal outcomes across the adult population and types of diabetes.
  • Apply the principles of behaviour change to empower patients to make sustainable lifestyle changes that improve their diabetes control, reduce complications, and enhance quality of life.

Key Requirements:
  • NVQ/Diploma Level 4 in Health and Social Care or equivalent level of knowledge, skills, and experience.
  • Experience of completing patient assessments in a variety of settings including patients' own home or Treatment Room.
  • Knowledge of planning and completing existing patient support assessments, awareness of insulin-dependent diabetes, and clear understanding of supporting and guiding patients with aspects of their care.
Salary Range: £28,000 - £35,000 per annum
