Health and Wellness Facilitator

This is an exciting opportunity to join the NHS team as a Health and Wellness Facilitator. As an Optimising Care Facilitator, you will work closely with patients to provide tailored education, guidance, and behaviour change interventions to support them in achieving better health outcomes.
Key Responsibilities- Wellness Programmes: Deliver high-quality diabetes education to patients, their families, and colleagues, ensuring a holistic approach to care. Facilitate group and individual educational sessions on topics such as medication management, blood glucose monitoring, nutrition, exercise, and stress management.
- Patient Support: Provide support for Diabetes Care: Deliver support, education, and advice to individuals living with diabetes, ensuring optimal outcomes across the adult population and types of diabetes.
- NVQ/Diploma Level 4 in Health and Social Care or equivalent level of knowledge, skills and experience.
- Care Certificate or willingness to undertake this within induction period.
Estimated Salary: £38,000 - £48,000 per annum.
