Responsibilities:

The Nursing Home Care Coordinator will:

  • Develop and implement individualized care plans to meet the unique needs of each resident
  • Collaborate with healthcare professionals to ensure seamless transitions of care
  • Provide emotional support and guidance to residents and their families

About You:

To be successful in this role, you will need experience working in a nursing home or similar setting, excellent communication skills, and a compassionate approach to patient care. A positive attitude, empathy, and strong interpersonal skills are essential in delivering high-quality care and support.

Salary: £20,500 per annum (dependent on experience)
