About the Role

We are seeking a highly skilled and compassionate Nursing Associate to join our Dementia Outreach Service at Southern Health NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality care to individuals living with dementia and their families.

Key Responsibilities
  • Conduct comprehensive assessments of individuals' needs and risks, ensuring timely and effective interventions.
  • Develop and implement personalized care plans, working collaboratively with multidisciplinary teams and service users.
  • Provide expert guidance and support to care facilities, families, and individuals, promoting person-centered care and empowering them to make informed decisions.
  • Engage in proactive engagement with people at risk of hospital admission, working closely with care facilities, GP liaison, and Adult Social Care to prevent unnecessary hospitalizations.
  • Collaborate with the team to develop and implement effective referrals, GP liaison, and family support, ensuring seamless care transitions.

Requirements
  • RMN or Allied Health Professional (Mental Health) qualification.
  • Experience in assessment, risk management, crisis intervention, and therapeutic engagement.
  • Knowledge of models of practice in social and health care, as well as interventions and strategies that address social and health care needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Ability to work flexibly, including evenings and weekends, and to travel to various locations as required.
