If so, we have an exciting opportunity for you to work in our Dementia Outreach Service, based within the Integrated Mental Health Hub at St. Mary's Hospital on the Isle of Wight.

We are looking for an enthusiastic person with a passion for delivering excellent care to people living with Dementia to join the work of our team.

The team aims to proactively engage with people who have a dementia and are at risk of further deterioration, which may lead to a hospital admission out of area. We work closely with care facilities, people and families in their own homes, GP liaison and Adult Social Care and our NHS Memory Service to prevent admissions.

We do this by providing short term intensive support, which includes hands-on care, giving advice and support to care facilities, creating effective referrals, GP liaison and family support. We also complete cognitive testing, mental state assessments, and physical screening.

If we cannot prevent hospital admission, we seek to engage with mainland hospitals for regular updates, CPA reviews and weekly MDT meetings.

Applicants for this post must be flexible and available across 7 days between 8am-6pm.

Main duties of the job

* To ensure and take responsibility for the provision of comprehensive assessments of needs and risks of people referred to the service.

* To take responsibility for planning and the delivery of care to people requiring an acute and intensive response from this service.

* To plan and take responsibility in collaborating with service users and their carers together with other services/agencies on how people's needs are going to be met in short-term and long-term situations.

* To be active in the development of the service and continued development of professional practice and role.

* To ensure that relationships and own behaviour are managed in line with the Trust's Equalities Framework competencies.

* To ensure all communications are undertaken in a sensitive, confidential and professional manner.

* Interdisciplinary working and liaison with internal and external agencies to promote holistic and person-centred care and inclusion

Qualifications

Essential

RMN or Allied health professional (Mental Health) Evidence based and experience of reflective practice. Current national legislative and professional requirements relating to social and health care and processes implemented locally (Community Care Act, Mental Health Act)

Experience

Essential

Assessment of risk, crisis management, intervention and management. Experience in therapeutic engagement to patients in groups. Wide knowledge/ appreciation of a range of models of practice in social and health care. Knowledge of interventions and strategies which embrace social and health care needs. Report writing Risk assessment and management. Mentorship /supervision qualifications/training Engagement Solution Focused working Liaison/negotiation Decision making
