About the Role
Southern Health NHS Foundation Trust is seeking a highly skilled and compassionate individual to join our Dementia Outreach Service as a Nursing Associate - Dementia Services Specialist. This exciting opportunity will enable you to make a real difference in the lives of people living with dementia and their families.
Key Responsibilities
- Conduct comprehensive assessments of needs and risks of people referred to the service, ensuring timely and effective interventions.
- Develop and implement care plans in collaboration with service users, carers, and other healthcare professionals, promoting holistic and person-centered care.
- Work closely with the multidisciplinary team to provide short-term intensive support, including hands-on care, advice, and support to care facilities, and effective referrals.
- Engage with people who have dementia and are at risk of further deterioration, preventing hospital admissions and promoting community-based care.
- Collaborate with mainland hospitals for regular updates, CPA reviews, and weekly MDT meetings, ensuring seamless care coordination.

Requirements
- RMN or Allied Health Professional (Mental Health) qualification.
- Experience in assessment, risk management, and crisis intervention.
- Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
- Ability to work flexibly, including evenings and weekends, to meet the needs of service users.
