Job summary

The Memory Assessment Services are a community team who are looking to recruit an enthusiastic Band 6 Registered Mental Health Nurse in the role of a Nurse Assessor who has a passion for working with people living with Dementia. The Team has gone through a recent Transformation Process and is currently working with Community Partners in a Whole Person Approach.

The Role of a Nurse Assessor was recognised within the transformation process as a vital part of the team, in order to deliver excellent care to the people we support, pre and post assessment.

As part of the Memory Assessment Service, you will be working within a multi-disciplinary team that is consultant led and practitioner delivered. Your expertise as an Nurse Assessor will be valued and integral to the development of the services to support people living with Dementia including Young Onset Dementia (people under the age of 65).

The successful candidate will join a team that is committed to ensuring that the person under the care of the Memory Assessment Service will receive excellent and compassionate care. Clinical activities take place primarily in people's homes and can include other community settings such as care homes and other supported living residences.

Main duties of the job

As a member of our team, you will be based within the Memory Assessment Service and will work within a Multi-Disciplinary Team that includes Specialist Occupational Therapists, Nurse Consultant, Advanced Clinical Practitioner, Nurse Assessors, Consultant Psychiatrists, Associate Practitioners with an efficient administrative support team to provide a multi-disciplinary service for people with dementia, their families and/or their Carers. In addition to the Memory Assessment Services, you will work in partnership with The Admiral Nurses and other Community Services. The role will include working autonomously as well as collaboratively within the wider team.

Qualifications

Essential

RMN or Allied health professional (Mental Health). Evidence based and experience of reflective practice post qualification. Mentorship/supervision qualifications/training.

Experience

Essential

Assessment of risk, crisis management, intervention and management. Wide knowledge/appreciation of a range of models of practice in social and health care. Knowledge of interventions and strategies which embrace social and health care needs. Current national legislative and professional requirements relating to social and health care and processes implemented locally (The Care Act 2014, The Mental Capacity Act 2005, Mental Health Act 1983) Report writing, Risk assessment and Risk management. Good communication and engagement skills, Solution Focused, Liaison/negotiation and Decision-making.

To undertake lone working to patients with mental health concerns. Ability to work over a range of shifts covering a seven-day period. Car driver and clean driving licence.
