Key Responsibilities- Conduct comprehensive assessments of individuals' needs and risks, ensuring timely and effective interventions.
- Develop and implement personalized care plans, working collaboratively with multidisciplinary teams and service users.
- Provide expert guidance and support to care facilities, families, and individuals, promoting person-centered care and empowering them to make informed decisions.
- Engage in proactive engagement with people at risk of hospital admission, working closely with care facilities, GP liaison, and Adult Social Care to prevent unnecessary hospitalizations.
- Collaborate with the team to develop and implement effective referrals, GP liaison, and family support, ensuring seamless care transitions.

Requirements- RMN or Allied Health Professional (Mental Health) qualification.
- Experience in assessment, risk management, and crisis intervention.
- Strong therapeutic engagement skills, with the ability to work effectively in groups.
- Knowledge of various models of practice in social and health care, with a focus on person-centered care.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with multidisciplinary teams.
- Ability to work flexibly, including evenings and weekends, to meet the needs of service users.
