Community Health Nursing Position

We are seeking an experienced and skilled Community Health Nurse to join our team in South West London. As a General Practice Nursing Role, you will play a crucial part in delivering high-quality patient care and contributing to the success of our GP surgery. This is an ideal opportunity for someone with strong experience in managing Long-Term Conditions such as Asthma, COPD, Diabetes, and Hypertension.
Responsibilities:
- Deliver core Practice Nurse services, including health checks, cervical screening (smears), dressings, wound management, immunisations, and travel vaccinations.
- Manage Long-Term Conditions, achieving high QOF standards and using the Ardens Template where preferred.
- Provide expert advice and guidance to patients, carers, and healthcare professionals, promoting healthy living and disease prevention.
Flexible Schedule: We offer a flexible working arrangement, requiring two 4-hour shifts on weekdays and a 3-hour Saturday remote clinic (9:00 am - 12:00 pm). At least two face-to-face sessions per week are mandatory, with the option to work up to 5 sessions per week. The start date is negotiable, but we are looking for a candidate to begin in March/April 2025.
Requirements: To succeed in this role, you must have experience in managing Asthma, COPD, Hypertension, and Diabetes.
