Job Summary:

We are recruiting for a highly skilled Nurse Practitioner Community Health professional to join our Community Services Institute Inc team in West London. As a Band 5 Community Nurse, you will be responsible for delivering comprehensive community health services to patients in their homes or communities.

Key Responsibilities:
- Conducting home visits and assessing patients' health needs
- Developing and implementing individualized care plans in collaboration with other healthcare professionals
- Providing education and support to patients and their families on healthy living and disease management
- Facilitating communication between patients and healthcare providers

Requirements:
- Current NMC registration as a qualified nurse
- A minimum of 6 months of experience working in community healthcare
- Enhanced DBS clearance within the last year
- Car ownership and ability to travel for work

- Excellent hourly rates of pay, ranging from £25-£35 per hour
