Nurse Practitioner in Community Healthcare

Central London Community Healthcare (CLCH) is a leading provider of community healthcare services in London and Hertfordshire. We are seeking a skilled Nurse Practitioner to join our team.
About the Role:
As a Nurse Practitioner, you will work closely with patients and their families to develop and implement tailored health plans. You will also collaborate with other healthcare professionals to ensure seamless delivery of services.
Responsibilities:
- Conduct comprehensive health needs assessments and write care plans
- Work collaboratively with patients and their families to achieve optimal health outcomes
- Collaborate with other healthcare professionals to ensure efficient service delivery
- Develop and implement training programs for staff members
Requirements:
- Masters or Bachelors degree in Nursing or related field
- Current registration with NMC
- Post-registration experience in community nursing
- Basic computer skills
