An exciting opportunity has arisen for a highly motivated individual to join our Community Nursing Team covering the Shrewsbury North Team locality in the provision of planned care to patients in the community.

About the Role

We are looking for a caring, motivated, and friendly Community Nurse to work collaboratively with other healthcare professionals in delivering high-quality patient care. The ideal candidate should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care.

Main Duties
  • You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for ongoing professional development.
  • Provide nursing care to patients within their own homes or chosen place of residence, offering an alternative to hospital admission, supporting timely discharge, and enabling people to realize their potential.
About Us

Shropshire Community Health NHS Trust provides community-based health services for adults and children in Shropshire, Telford and Wrekin, and some services in surrounding areas too. These range from district nursing, health visiting, and running four community hospitals through to providing very specialist community care through talented and dedicated staff.

Requirements

To be successful in this role, you will need:

  • RGN NMC Registered.
  • Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes.
  • Post registration general experience.
  • Experience of working in a multi-disciplinary / multi-agency health/social care environment.
