Community Health Nurse

To deliver high-quality nursing care to patients in their homes and provide rapid response, rehabilitative, and post-acute care in community settings. You will be part of a team that operates through agreed shift patterns, spanning 8am to 10pm, 7 days a week.
Key Responsibilities- Delivering Skilled Care: Provide effective and evidence-based nursing care to patients in various community settings.
- Assessing Needs: Utilize specialist skills and knowledge to assess the physical and psycho-social needs of patients and develop a comprehensive care plan.
- Managing Risk: Make risk assessments to ensure patients can safely stay at home or return home from another care setting, providing necessary nursing and supportive packages.
- Providing Autonomous Care: Offer clinical care to patients with undifferentiated, undiagnosed, or complex health needs and long-term conditions.
