About the Role:

We are seeking a Disability Support Coordinator to join our team at Leonard Cheshire UK. As a Disability Support Coordinator, you will play a key role in supporting individuals with disabilities to achieve their goals and live independently in their own homes.

Job Summary:

The successful candidate will be responsible for coordinating support services, including personal care, medication, and transportation. You will also work collaboratively with our Person Centred approach to support individuals in achieving their goals and aspirations.

Key Responsibilities:

  • Coordinate support services, including personal care, medication, and transportation
  • Work collaboratively with individuals to support them in achieving their goals and aspirations
  • Maintain accurate records of individual's progress and needs
  • Communicate effectively with stakeholders, including family members and healthcare professionals

Requirements:

To be successful in this role, you will need to have excellent communication skills, be able to work flexibly, and have a patient and understanding approach. A personal care qualification is desirable but not essential.
