About the Role
As a Care Coordinator, you will be responsible for developing and implementing tailored support plans, working closely with multidisciplinary teams, and ensuring seamless transitions between services. Your role will involve coaching and mentoring colleagues, promoting positive behavior, and maintaining accurate records. To succeed, you will need 2-4 years' experience in a similar setting, a strong understanding of autism and learning disabilities, and excellent communication skills. We offer a competitive salary range of £28,500 - £34,500 per year and a comprehensive benefits package.
