About the Role

We are looking for an exceptional Care Coordinator to join our adult mental health team. As a Care Coordinator, you will be responsible for assessing, planning, and implementing care plans for adults with mental health needs. This is a critical role that requires strong clinical skills, excellent communication, and interpersonal skills.

You will work closely with our multidisciplinary team to ensure that patients receive the best possible care and support. The ideal candidate will have experience of working with adults with mental health needs and will possess the ability to work effectively as part of a team.

The salary for this role is approximately £35,000 - £45,000 per annum, depending on experience. You will also receive comprehensive training and development opportunities to help you grow in your career.
