About the Role
We are seeking a dedicated Care Coordinator to join our team at Lincolnshire County Council. In this role, you will work within a multi-disciplinary service supporting individuals with learning disabilities and care needs.
As a Care Coordinator, you will be responsible for managing an allocated caseload of clients, reviewing current DoLS orders, and discussing agreements with relevant stakeholders to ensure care remains in the individual's best interests.
You will also complete DoLS application paperwork, maintain case notes, and communicate effectively with families and advocacy services.
This is a rewarding opportunity to make a positive impact on the lives of vulnerable adults, and we are looking for someone with excellent communication skills, creative thinking, and problem-solving abilities.
Main Duties:
- Manage an allocated caseload of clients from across the LD service.
- Review current DoLS orders and discuss agreements with relevant stakeholders.
- Complete DoLS application paperwork and maintain case notes.
- Communicate effectively with families and advocacy services.

Salary and Benefits
We offer a competitive salary of £32,000 - £40,000 per annum, depending on experience, plus a generous pension scheme, reimbursement for professional registration fees, career progression opportunities, and annual leave entitlement.
About You
To succeed in this role, you will need to hold a Social Work or Nurse qualification and have registration with SWE/NMC in date. We are looking for someone with a positive approach, self-motivation, enthusiasm, and resilience.
