Job Title: Community Health Navigator for Addiction Recovery

We are seeking a highly motivated and empathetic Community Health Navigator for Addiction Recovery to join our team. The successful candidate will provide personalized support and guidance to individuals and families affected by addiction, helping them navigate the recovery process.

This is a full-time, permanent role that operates within the framework of our Inclusion Services department. You will work closely with multidisciplinary teams to develop and implement effective care plans, improving the quality of life and fostering positive change in individuals affected by addiction.

Main Responsibilities:
  • Provide personalized support and guidance to individuals and families affected by addiction.
  • Work closely with multidisciplinary teams to develop and implement effective care plans.
  • Evaluate evidence-based practice and ensure that clinical work is up-to-date and in accordance with current policy and legislation.
  • Deliver training and education to healthcare professionals on best practices in addiction treatment.

Inclusion Services, part of Midlands Partnership University NHS Foundation Trust, supports individuals, families, and communities affected by addiction, crime, mental health, or need treatment for their sexual health.

Requirements:
  • Adhere to professional codes of conduct, including accountability, confidentiality, and commitment to high-quality service.
  • Maintain the security of all records and information while adhering to Trust and Partner agencies' confidentiality policy.
  • Lead community outreach initiatives to engage with individuals and families affected by addiction.
  • Develop and deliver training programs for healthcare professionals on evidence-based practices in addiction treatment.
Experience:
  • Proven experience in community outreach and engagement roles.
  • Developing and maintaining successful partnerships with GPs, treatment agencies, and colleagues within the community.
  • Clinical risk assessment and management of vulnerable service user groups.
Salary:

The salary for this position ranges from £37,338 to £44,962 per annum.
