About the Role

We are seeking a highly skilled and dedicated Therapy Support Worker to join our Community Health and Social Care team in Barnstaple, North Devon. As a key member of our team, you will play a vital role in supporting patients with a range of disabilities and acute or long-term conditions.

Key Responsibilities

  • Support patients with mobility and home exercise programs, small equipment aids, and functional tasks around the home.
  • Work with patients and caregivers to achieve goals that are important to the patient, such as being able to walk with no assistance around their home or being able to make a snack in the kitchen while experiencing reduced strength.
  • Document all contacts and maintain accurate records.
  • Communicate regularly with other health and social care professionals and wider partners to support patients and their presenting needs safely within their own home/environment.

Requirements

  • Experience of working in a care or rehabilitation setting.
  • Good communication skills, a proactive and practical approach, and good organisational skills.
  • Ability to work independently and as part of a team.
  • Knowledge of patient-centered care and rehabilitation principles.
