About the Role

We are seeking a highly skilled and compassionate Therapy Support Worker to join our Community Health and Social Care team in Barnstaple, North Devon.

The successful candidate will work closely with our multidisciplinary team to provide high-quality patient care and support, focusing on rehabilitation and enabling individuals to reach their full potential.

Key Responsibilities:

  • Support patients with a range of interventions, including mobility and home exercise programs, small equipment aids, and functional tasks around the home.
  • Work collaboratively with registered therapy team members to develop and implement patient care plans.
  • Provide emotional support and guidance to patients and their families, promoting independence and confidence.
  • Contribute to the development and maintenance of a safe and supportive environment for patients and staff.
  • Participate in regular team meetings and contribute to the development of the team's goals and objectives.

Requirements:

  • Experience of working in a care or rehabilitation setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Commitment to providing high-quality patient care and support.
