We are seeking a highly motivated and experienced Therapy Support Professional to join our Community Health and Social Care team. As a key member of our team, you will play a vital role in supporting individuals with a range of disabilities and acute or long-term conditions.
Key Responsibilities:
- Provide support to patients with a range of interventions, including mobility and home exercise programmes, small equipment aids, and functional tasks around the home.
- Work closely with registered therapy team members to develop and implement patient care plans.
- Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care delivery.
- Maintain accurate and up-to-date records of patient interactions and progress.
- Collaborate with other healthcare professionals and agencies to ensure comprehensive patient care.
