Job Summary

We are seeking a highly skilled and compassionate Therapy Support Professional to join our Community Health and Social Care team in Barnstaple, North Devon. As a key member of our team, you will play a vital role in supporting individuals with a range of disabilities and acute or long-term conditions.

Main Responsibilities
  • Support patients with mobility and home exercise programs, small equipment aids, and functional tasks around the home.
  • Work collaboratively with registered therapy team members to develop and implement patient care plans.
  • Communicate effectively with patients, caregivers, and other healthcare professionals to ensure seamless care coordination.
  • Document patient interactions and progress in our electronic patient recording system.
  • Participate in regular team meetings and contribute to the development of new ideas and initiatives.
Requirements
  • Experience working in a care or rehabilitation setting.
  • Excellent communication and interpersonal skills.
