Clinical Care Specialist

We are seeking an experienced Clinical Care Specialist to join our team at Chesterfield Royal Hospital NHS Foundation Trust. The ideal candidate will have a strong understanding of long-term conditions and the ability to provide expert care to patients who are frail and/or with multiple long-term conditions.
Job Summary:
- To provide enhanced clinical management for patients who are frail and/or with multiple long-term conditions, working in the community to prevent hospital admissions and improve quality of life.
- Investigate and diagnose unwell individuals with frailty and/or long-term conditions in their own homes and in care homes.
Main Responsibilities:
- Assessing patient needs and developing personalized care plans.
- Providing high-quality care to patients and families, including managing complex medical conditions.
- Working closely with multidisciplinary teams to ensure seamless care delivery.
- Participating in education and training programs to enhance clinical skills and knowledge.
