Clinical Care Coordinator

We are seeking a Clinical Care Coordinator to join our team at Chesterfield Royal Hospital. This is a fantastic opportunity to work in a dynamic and supportive environment, where you will have the chance to make a real difference in the lives of our patients.
About the roleThis is a full-time permanent position, working 37 hours per week. The successful candidate will have experience of working in a community setting and be qualified as a Registered Nurse or have an equivalent qualification. You will also have completed both level 7 modules of APACS/CDM and Non-Medical Prescribing qualification and completion of E-portfolio to be uplifted from a trainee band 6 to a qualified band 7.
Main responsibilities- To provide expert enhanced clinical management for patients who are frail and/or with multiple long-term conditions who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission, or unnecessary length of hospital stay.
- Investigate and diagnose an unwell individual with frailty and or long-term conditions in their own homes and in care homes.
- High quality care to patients and families.
- Managing complex medical conditions including long-term conditions and frailty.
- Dealing with challenging behaviour.
We offer a competitive salary of £45,500 - £62,700 per annum, depending on experience, and access to a range of benefits, including a pension scheme, annual leave and flexible working arrangements.
