Acute Assessment Service Coordinator Job Description

We are seeking a highly skilled and experienced Acute Assessment Service Coordinator to join our Clinical Assessment Service at Care Plus Group. As a key member of our team, you will coordinate the assessment of patients in acute need and work closely with healthcare professionals to ensure the best possible outcomes.

Key Responsibilities
  • Coordinate the assessment of patients in acute need
  • Work closely with healthcare professionals to ensure the best possible outcomes
  • Undertake initial health and social care screening over the telephone
  • Respond to referrals into the service
  • Develop and maintain strong relationships with patients, relatives, and healthcare professionals
Salary and Benefits

The estimated salary for this position is £27,000 - £34,000 per annum, depending on experience. In addition to a competitive salary, you will receive access to training and development, ongoing supervision and support, and the opportunity to contribute to the future development of the service.
