Care Plus Group is a leading provider of NHS community health and social care services in North East Lincolnshire. We are seeking an experienced Health and Social Care Support Professional to join our Clinical Assessment Service (CAS) team.

The CAS provides urgent care triage for patients across the region, receiving referrals from healthcare professionals and responding to emergency calls. As a key member of our team, you will be responsible for assessing patient needs, providing guidance on care options, and working collaboratively with multi-disciplinary teams to ensure the best possible outcomes.

About the Role

  • Provide telephone-based assessments and advice to patients, relatives, and healthcare professionals
  • Collaborate with multidisciplinary teams to develop and implement care plans
  • Work under the direction of senior practitioners to ensure high-quality care delivery
  • Contribute to the development and improvement of service protocols and procedures

Requirements

  • NVQ Level 3 in Customer Care or Health and Social Care or equivalent
  • 2 years previous experience of working in a health and/or social care environment
  • Ability to work under pressure, displaying patience, tact, and diplomacy
  • Ability to assimilate information quickly and effectively
  • An understanding of and respect for confidentiality in relation to all aspects of work
