About the Role

We are seeking a highly skilled and motivated Adult Mental Health Practitioner to join our multidisciplinary team. As a key member of our community team, you will work closely with service users, families, and carers to provide high-quality care and support.

Key Responsibilities
  • Assess and manage the mental health needs of service users, developing and implementing effective care plans.
  • Provide skilled interventions, including risk management and crisis intervention, to ensure the safety and well-being of service users.
  • Supervise and support other team members, promoting a culture of collaboration and teamwork.
  • Establish and maintain effective relationships with service users, families, and carers, promoting open and honest communication.
  • Contribute to the development and implementation of service policies and procedures, ensuring compliance with statutory requirements.
Person Specification
  • Registered Mental Health Practitioner with current NMC registration.
  • Significant experience of working within a mental health setting, with a proven track record of assessing and managing risk.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Ability to promote and develop links with other professionals, including primary care and long-term conditions teams.
