About the Role

We are seeking a highly skilled and motivated Adult Mental Health Practitioner to join our multidisciplinary team. As a key member of our community team, you will work closely with service users, families, and carers to provide high-quality care and support.

Key Responsibilities
  • Assess and manage the mental health needs of service users, developing and implementing effective care plans.
  • Provide skilled interventions, including risk management and crisis intervention, to ensure the safety and well-being of service users.
  • Supervise and support other team members, promoting a culture of excellence and continuous learning.
  • Establish and maintain effective relationships with other agencies and professionals, ensuring seamless care coordination.
  • Stay up-to-date with best practices, research, and statutory codes of practice, applying this knowledge to improve service delivery.
Person Specification
  • Assessment and care planning skills, with experience in risk assessment and management.
  • Relevant post-registration qualifications and/or training, with a commitment to ongoing professional development.
