About the Role

We are seeking a highly skilled and motivated Adult Mental Health Practitioner to join our Forensic Outreach Liaison Service (FOLS) team. As a key member of our multidisciplinary team, you will provide effective clinical input and support to individuals with complex mental health needs, including those with learning disabilities and autism spectrum disorder.

Key Responsibilities
  • Assess and manage risk in a variety of settings, including secure and community environments.
  • Develop and implement holistic care plans that address the bio-psycho-social needs of service users.
  • Provide skilled interventions, including therapeutic and supportive services, to individuals with complex mental health needs.
  • Work collaboratively with colleagues and partners to ensure seamless transitions and effective care coordination.
  • Contribute to the development and implementation of policies and procedures that promote best practice and quality care.
Requirements
  • RMN/RNLD registration with the NMC.
  • Experience of working with complex mental health needs, including learning disabilities and autism spectrum disorder.
  • Ability to assess and manage risk in a variety of settings.
  • Excellent communication and interpersonal skills.
  • Knowledge of equality and diversity principles and their application in care planning and delivery.
