About the Role

We are seeking a highly skilled and motivated Adult Mental Health Practitioner to join our Forensic Outreach Liaison Service (FOLS) team. As a key member of our multidisciplinary team, you will provide effective clinical input and support to individuals with complex mental health needs, including those with learning disabilities and autism spectrum disorder.

Key Responsibilities
  • Assess and manage risk in a variety of settings, including secure and community environments.
  • Develop and implement holistic care plans that address the bio-psycho-social needs of service users.
  • Provide skilled interventions, including therapeutic and supportive approaches, to promote recovery and well-being.
  • Work collaboratively with colleagues and partners to ensure seamless transitions and effective care coordination.
  • Contribute to the development and implementation of policies and procedures that promote equality and diversity.
Requirements
  • RMN/RNLD registration with the NMC.
  • Experience of working with complex mental health needs, including dual diagnosis and personality disorder.
  • Ability to assess and manage risk in a variety of settings.
  • Excellent communication and interpersonal skills.
  • Knowledge of equality and diversity principles and practices.
