We are seeking a highly motivated and experienced Adult Mental Health Practitioner to join our community team. As a key member of our team, you will be responsible for providing high-quality care and support to individuals with moderate to severe mental health issues.
Key Responsibilities- Assess and manage the mental health needs of service users, developing and implementing effective care plans.
- Work collaboratively with other professionals and agencies to ensure seamless care and support.
- Provide support and guidance to service users, carers, and families, promoting health and well-being.
- Contribute to the development and implementation of risk management protocols and procedures.
- Supervise and support other team members, promoting a culture of excellence and continuous improvement.
- Current registration with the NMC (RMN) or Social Work England/HCPC (Social Worker/Occupational Therapist).
- Significant experience of working within a mental health setting, with a focus on community-based care.
- Excellent assessment and care planning skills, with the ability to manage risk and promote health and well-being.
- Strong communication and interpersonal skills, with the ability to work collaboratively with others.
- A current driving licence and access to a car during working hours (reasonable adjustments will be considered for applicants with disabilities).
